Toyin Ajayi

Co-Founder, Cityblock Health

Dr. Toyin Ajayi is co-founder and CEO of Cityblock, an outcomes-based healthcare provider for Medicaid and dually eligible beneficiaries in underserved communities. Cityblock’s integrated and tech-enabled care model meets individuals where they are, delivering highly personalized medical care, behavioral health care, and social services. 

A member of the National Academy of Medicine, Dr Ajayi has been named to Inc.’s  Female Founders 500 list, TIME100 Next list, Modern Healthcare’s Top Women Leaders in Healthcare list, STAT’s STATUS List, and is part of the Aspen Institute’s 2023 class of Henry Crown Fellows. Dr. Ajayi serves on the Board of Directors of Evolent Health (NYSE: EVH), and on the Board of Foodsmart. As co-founder of Coalition Partners, a venture capital firm focusing on early-stage companies, she actively invests in and mentors other entrepreneurs.

Dr. Ajayi received her undergraduate degree from Stanford University, an MPhil from the University of Cambridge, her medical degree, with Distinction in Clinical Practice, from King’s College London School of Medicine, and in 2024 was awarded an honorary Doctorate of Science from Georgetown University. Board certified in Family Medicine, Dr. Ajayi completed her residency training at Boston Medical Center and practiced as a hospitalist and primary care provider with a focus on patients with chronic, complex and end-of-life needs.
